Payments and  Insurance

Therapy sessions are  scheduled for 45- 50 minutes. 
I am a preferred or participating provider with the following plans: BCBS, TRICARE,  Magellan,  Medicare, EBMS, AETNA, Military One Source and others, including most plans covering Federal Employees.  IF YOU ARE AN ACTIVE DUTY MILITARY MEMBER, YOU WILL NEED A REFERRAL FROM YOUR PCM.  MILITARY DEPENDENTS DO NOT, AND CAN SCHEDULE WITH ME DIRECTLY. 

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?
  • What is my deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is authorization required?

Payment
 Payment is due at the time of service for all co-pays, deductIbles, and cost-shares as determined by your insurance plan or in full if you are not covered by an insurance or EAP plan. 

Cancellation Policy
If you do not show up for your scheduled therapy appointment and you have not notified me at least 24 hours in advance, you will be required to pay a late cancellation/No Show Fee of $100. Your appointment time is reserved just for you, and without proper notice the time can not be filled. Of course, I will work with you in the event of illness or emergency.

If you are using Medicare or a commercial insurance with a deductible, please contact your carrier to find out what your financial responsibility is, i.e. deductible, copay or co-insurance. That is determined by them and must be collected by me.  For first visits, if you have traditional health insurance with a deductible and copay/coinsurance, and do not have the information from your insurance company indicating what your part is, you will be asked to provide a Credit/Debit card authorization so that once your insurance has paid their part, your balance may be charged to you, via your card on file. 

If YOU are an Active Duty Military member, please have a referral from your PCM. 

If you are using an EAP, PLEASE have the name of the EAP company (not your employer but:  Magellan, EAP Consultants, Military One Source) AND your authorization number. 

When you arrive, please have a seat and I will come and get you at the time of your appointment. I do not have a receptionist. There is no need to come early, as your paperwork will have  been completed. 

And, most importantly, be proud of yourself for taking this step towards self-care.
